About the project
The project “Building an Ecosystem of Participation and Support for Aftercare Youth in Ostrobothnia – JOTEP” aims to strengthen support for young people transitioning from aftercare to independent adulthood. Young people in aftercare often face challenges such as lower educational attainment, limited employment opportunities, mental health issues, and weaker social networks. By developing the structure of aftercare services and creating a holistic, multidisciplinary support model, the project seeks to ensure that young people receive the tools they need to manage daily life and work toward their educational and employment goals.Project goal
The main objective of the project is to reform aftercare so that young people receive stronger participation, more individualized support, and smoother transitions within service systems. The project will build a functional ecosystem that supports the transition to independence through clear operational models, measurable services, and data-driven development. Another key goal is to strengthen the competence of professionals through training and guidance, enhance multidisciplinary collaboration, and create methods that better address the real needs of young people. As a result, young people will receive higher-quality and more tailored support, while professionals gain improved tools to provide effective and impactful services.
